WINNICK PATIENT ADVOCACY PRIVACY POLICY

Winnick Patient Advocacy (WPA) is committed to personal information privacy.  This policy describes how WPA collects, uses, and discloses personal information.

WPA has drafted this policy in accordance with the Electronic Communications Privacy Act of 1986, 18 U.S.C. 2510, as amended by the Electronic Communications Privacy Act of 2000, H.R. 5018, and in accordance with the Health Insurance Portability and Accountability Act of 1986 (HIPAA), and the Personal Information Protection and Electronic Documents Act, S.C. 2000, c. 5 (PIPEDA), and any other relevant U.S. (Federal/ State) or Canadian (Federal/ Provincial) legislation.

What Personal Information Do We Collect?

  •  contact information such as name, address, telephone number, and e-mail address;

  •  health care history, data, requests and needs;

  •  service related information such as the services we provide to you; and

  •  client relationship information such as client requests and feed-back and information requested or provided by or from you.

Why Do We Collect Your Personal Information?

We collect your personal information in order to:

  •  help us serve you;

  •  get you the care you need when you need it;

  •  help physicians, nurses, other health care providers, and health insurance companies help you;

  •  facilitate research so we can help you to make informed decisions.

Your Consent Is Important To Us

Your consent may be implied or express.  Express consent can be given orally, electronically, or in writing.  Implied consent can be inferred from your action(s) or inaction(s).  For example, if you give us your personal health care information, it can be inferred that you wish us to use this information to help you.   

If you provide us with personal information by electronic or other means (including, but not limited to telephone, mail, or fax) you give us the right to communicate with you by electronic means or otherwise.  We will then share this information with other parties (including, but not limited to) physicians, nurses, hospitals, other health care providers, insurance companies, any of our employees, contractors, consultants, and other parties who require such information to assist us with establishing, maintaining and managing our relationship with you, including:  third parties that provide services to us on our behalf; third parties that collaborate with us in the provision of services to you; and third parties whose services we use to perform our services.

In addition, personal information may be disclosed or transferred to another party in the event of a change in ownership of, or grant of a security interest in, all or part, or WPA, provided such parties use or disclose your information in a manner consistent with this privacy policy.

Your information may be forwarded by electronic or other means across State/ Provincial, Federal, and international boundaries.  Information may be provided, without your consent  to Federal/ State (U.S.) or Federal/ Provincial (Canadian) authorities, as required by law.

Protecting Personal Information

We understand the importance of protecting your information.  As such, we do all we can through physical, and procedural means to protect your information.

Revisions and Interpretation

WPA may, from time to time, make changes in this Policy.  We will post any revised versions of this Policy on our web site at www.patient-health-advocacy.com.  The rights and obligations contained in this Policy may not be available to all individuals or in all jurisdictions.  This Policy does not create or confer upon any individual any rights, or impose upon WPA any right or obligations outside of, or in addition to any right or obligations imposed by United States’ Federal and State privacy laws, as, applicable.  Should there be in a specific case, any inconsistency between this Privacy Policy and the United States Federal and State privacy laws, as applicable, this Privacy Policy shall be interpreted, in respect of that case, to give effect to, and comply with, such privacy laws.
